Essential Duties and Responsibilities:

Home and Community Based Service - Community Integration, may provide the following supports to meet participants' habilitative outcomes as documented in the Individual Service Plan (ISP):

1. Services consist of support in the general areas of self-care, communication, fine and gross motor skills, mobility, personal adjustment, relationship development, socialization, and use of community resources.
2. Service also includes transportation services necessary to enable the participant to participate in the HCBS, in accordance with the participant's ISP.
3. Support that enables the participant to access and use community resources such as instruction in using transportation, translation and communication assistance related to a habilitative outcome, and services to assist the participant in shopping and other necessary activities of community life.
4. Support that assists the participant in developing or maintaining financial stability and security, such as plans for achieving self-support; general banking; personal and estate planning; balancing accounts; preparing income taxes; and record keeping.
5. Support that enables a participant to participate in community projects, associations, groups, and functions, such as support that assists a participant to participate in a volunteer association or a community work project.
6. Support that enables a participant to visit with friends and family in the community.
7. Support that enables a participant to participate in public and private boards, advisory groups, and commissions.
8. Support that enables the participant to exercise rights as a citizen, such as assistance in exercising civic responsibilities.
9. Support provided during overnight hours when the participant needs the habilitation service to protect their health and welfare. The individual staff providing the service must be awake.

Additional Responsibilities:

1. Submit accurate, completed progress notes every Monday by 12:00pm.
2. Complete monthly notes for each individual receiving HCBS services and turn them in the first Monday of every month.
3. Adhere to the Supervisory requirement and attend monthly face-to-face supervision.
4. Attend the annual ISP meetings for each individual supported.
5. Keep credentials updated with office personnel including: Driving License, Insurance, Registration, Criminal Clearances, and other credentialing necessary to complete the requirements of the position.
6. Attend 24 hours of training annually as well as initial pre-service training according the requirements set forth by OBRA
7. Complete necessary training based on the ISP of each individual.
