Kaleidoscope Family Solutions, Inc. provides community and home based services to persons with developmental and intellectual disabilities in Pawtucket, Providence, Cumberland, Lincoln, East Providence and Johnston areas.
Our team partners with family members and existing providers to create individualized goals allowing the team to help improve their life experiences.
RESPONSIBILITIES
The primary role of a Community Support Worker is to help individuals acquire and maintain skills to improve their daily-living, domestic, adaptive, and socialization skills.
* Provide 1:1 In-Home and Community Based support to individuals
* Assist individuals with achieving a variety of goals in the home and local community
* Transport individuals to medical appointments, personal shopping, and recreation activities
* Complete and Submit accurate weekly progress notes
* Complete necessary training based on the individuals' needs
REQUIREMENTS
* Minimal of a High School Diploma or General Equivalency Diploma (GED)
* Must have a valid driver's license and existing insurance, BCI
* Must be willing to drive individuals in personal car throughout the community
